Closing The Gap

Here at Your Health Griffith we are committed to the health of our Aboriginal and Torres Strait Islander community. Our practice participates in the Closing the Gap Program to better the health of our local community. Closing The Gap is a strategy that aims to reduce Indigenous disadvantage with respect to life expectancy, child mortality, access to early childhood education, educational achievement, and employment outcomes.

​Aboriginal and Torres Strait Islander people get more chronic diseases and get them a lot earlier than non-Indigenous Australians. The government has introduced programs to help many Aboriginal and Torres Strait Islander Australians better manage their own health, and especially chronic diseases.

There are two programs that Aboriginal and Torres Strait Islander people can sign up for:

1.  Closing the Gap Pharmaceutical Benefits (PBS) Co-payment measure: Provides you with access to low cost or free medications and will assist you to better manage or prevent illness.                                                      

2.  Practice Incentive Programs – Indigenous Health Incentive: Provides help for Aboriginal and Torres Strait Islander people aged 15 years and older with chronic diseases.  Chronic diseases include asthma, cancer, cardiovascular illness, diabetes, musculoskeletal conditions, stroke and other conditions. 

You will need to register each calendar year to receive the services and benefits.

Your doctor will also offer you a preventative health check to help identify and treat any health problems you may have. A health check provides your Doctor with important information about how you’re traveling health wise. Without knowing these things, it’s hard to look after your health and the health of your family, in the right way. The health check is free!
